Group Registration

Welcome to Attendee registration for CADCA's 36th National Leadership Forum!

Instructions for group discount: Groups of 5 or more receive a discount on the early-bird and advance registration rates only. All registrants must be a member to receive the group discount.

To qualify for the group rate:

1. All individuals must be associated with the SAME coalition/organization (no exceptions)
2. Coalition/organization must be an active CADCA member
3. Group must contain a minimum of 5 FULL-PAYING individuals (scholarship recipients & speakers do not count towards members of the group)
4. All members must be registered at the same time & pay in one transaction, using one form of payment.

Only use this form if you are CADCA member and are registering 5 or more individuals from the SAME ACTIVE organization. Cancelations that cause the group to drop below 5 will cause the remaining registrants to pay the individual rates. You must check the corresponding rate box for each adult and youth planning to attend.

*All individuals in the group must be listed under your organization's roster to receive the discounted rate*

Each individual (youth and adult) is required to register using their own unique email address.
*DO NOT register multiple individuals using the same email address*
Special Note: Adult Advisors will need to register an individual email for each of the youth leaders that will be attending Forum
