Register for CADCA’s 37th Annual National Leadership Forum using the form below. 

Attending only SAMHSA Prevention Day? Use the SAMHSA Prevention Day form.

Before You Start

  • Use a unique email address for each attendee
  • Have your payment method ready (credit card, check, or purchase order)
  • Confirm that your membership roster is up to date
  • Purchase order document (if paying by PO)
  • Signed consent form for participants under 18


Group Registration Requirements

Groups are only eligible for a discount on early bird and advance member registration rates. To qualify for group rates:
  • Organization must be an active CADCA member
  • Minimum of 5 full-paying attendees, and all individuals in the group must be listed under your organization's roster to receive the discounted rate
    • Need to add to your roster? Please visit this webpage or email membership@cadca.org
    • Scholarship recipients and speakers do not count towards members of a group
  • All attendees must be from the same organization or coalition
  • All registrations must be completed at the same time and paid in one transaction
  • You must use one payment method for the full group
If your group drops below five attendees, rates will be adjusted accordingly to the individual rate.

  • Exhibitors must register through the Exhibitor Service Center 
  • Speakers must register through the Speaker Service Center

Policies & Consent 

  • Substitutions are allowed at no cost.
  • If you have an outstanding balance with CADCA, your registration is subject to cancelation until the balance is paid.
  • No refunds; cancelations must be processed before February 1 to receive a 1-year credit towards a future CADCA Signature Event less a $150 admin fee.
    • All requests for registration cancelations must be submitted in writing via e-mail to events@cadca.org.
  • Cancelations received after February 1, conference no-shows, or failure to attend the training event will not receive a 1-year credit towards a future event.
  • By registering, you grant CADCA permission to include your name, organization, and state on the attendee list and to use your image captured in event photos/videos.
  • By registering, you also agree to CADCA’s registration policies, code of conduct, and waiver of liability.
